The same CPT code was billed twice at the same dollar amount for what was, by the treatment record, a single IV placement. Unless the chart documents two separate insertions, the second line item should be removed entirely.

Why this happens

Usually a data-entry artifact — one department bills the supply, another bills the procedure, and both charges make it onto the same itemized statement without anyone reconciling them.
