Most U.S. hospitals are 501(c)(3) nonprofits, which are legally required to maintain a written financial-assistance policy and make it widely known. Uninsured patients are often eligible for a substantial discount, sometimes full forgiveness, based on household income, but only if they know to ask.

Why this happens
Financial assistance policies exist, but hospitals aren’t always proactive about surfacing them. The burden typically falls on the patient to find and request the application before paying anything.
